• ACTis has raised three flags for SITA and placed two under monitoring.

• SITA’s institutional environment is currently vulnerable to informal influence and dysfunction.

• Persistent leadership instability within SITA – including frequent Managing Director (MD) changes, acting appointments and periods without a Board – has weakened oversight, strategic direction and internal controls. These governance weaknesses are exacerbated by a negative working environment, in which fear, victimisation and declining professionalism have reportedly constrained internal accountability. This increases the risk of irregular decision-making.

• Ongoing inadequate consequence management and failures by leadership to address investigation findings reflect weakened internal discipline and the normalisation of improper conduct. This has created an environment conducive to corruption and maladministration, prompting multiple investigations by authorities.

• Recurring audit qualifications from the Auditor-General of South Africa (AGSA), rising levels of irregular expenditure and an increasing number of Material Irregularities (MIs) point to systemic financial management failures rather than isolated lapses.

• SITA’s systemic dysfunction has led to a steady decline in performance over the past decade, reducing its capacity to fulfil its mandate. Multiple client departments, including the Ministries of Police, Home Affairs and Justice, have sought exemptions from using SITA’s services, citing their inefficiency, expense and unreliability.
